Once annually, every ROSS account holder must fill out either the FS-6600-7 or FS-6600-6 form. Please review the information below and determine which form is applicable to you. Once you have identified the correct form, please read the form, sign it, and provide the signed form to your dispatch office who will file and have it available for audit.
Please note: Overhead and Vendors with Web status only accounts do not need to sign one of these forms.
Statement of Employee Information Security Responsibilities: FS-6600-7
This form is for all FS employees and ADs with access to ROSS.
Statement of Information Security Responsibilities for Associate Users of Forest Service Systems: FS-6600-6
This form is for all Forest Service cooperators, volunteers, contractors, or other associates with access to ROSS.
